Our outpatient services provide rehabilitative, diagnostic and wellness services to patients who no longer need inpatient care. Patients may begin our outpatient program directly after their discharge from Drake Center or another hospital, or may begin at any time they can benefit. 

Our outpatient services include physical therapy, warm-water aquatic therapy, occupational therapy, speech-language pathology, spasticity and mobility, neurogenic incontinence, multiple sclerosis, lymphedema, wound, spine, community reentry, neuropsychological examinations, stroke recovery and diagnostic radiology services.

If you have a patient you think could benefit from one of these services, simply write a script for the appropriate treatment/therapy and fax to 513.418.2550 or ask the patient to bring the prescription when they arrive for therapy. Please include the following on your order:

• Patient name
• Patient date of birth
• Patient diagnosis
• Treatment/therapy being ordered
• Physician's signature
• Physician's printed name

If you are prescribing physical, occupational or speech therapy, you may prefer to complete our Outpatient Therapy Prescription Form. This can be done online, or by downloading, printing and faxing the completed document to 513.418.2550.

If you are prescribing neuropsychological testing for a medical diagnosis, we ask that you also complete our Neuropsychological Testing Intake Sheet. This can be done online, or by downloading , printing and faxing the completed document to 513.418.2550.

If you are prescribing outpatient wound care or HBO (Hyperbaric Oxygen) Treatment, you may prefer to complete our outpatient wound care prescription form. This can be done by downloading, printing and faxing the completed document to 513.418.2698.

If you are prescribing outpatient Reclast© infusion referral, you may prefer to complete our Reclast© Infusion referral form. This can be done online, or by downloading, printing and faxing the completed document to 513.418.2582.
